Membership Application -- Complete and submit the form to the Membership Chairperson, with a check payable to the GSDCW.
Win Form (DOC) -- Complete this MS Word fill-in form, then e-mail the form to the Historian. If your dog achieved a new title, please include a picture (which will be returned to you) for the Tales.
Win Form (PDF) -- Complete this PDF fill-in form, then print it and place it the bin in the Clubhouse or mail it to the Historian. If your dog achieved a new title, please include a picture (which will be returned to you) for the Tales.
